RCSD Daily Health Assessment Form

Dear RCSD Parents and Guardians,

The District is requiring families to complete a COVID-19 screening assessment with a temperature check and to fill out a health assessment form for every student who is entering one of the District’s school buildings.  For ease of use, and to replace wasteful and unhygienic paper forms, the District has purchased web-based software to host the form.

The link to access the form is below. It is compatible with any kind of web browser and can be accessed from a mobile device or on a desktop or laptop computer.

The link will also be posted to the homepage of the RCSD website under important messages and also be found on each school website under Families>Students, Parents, Guardians. 

Please complete the form each morning your child physically attends school. Please complete a separate form for each child. Middle and High School families must complete the health assessment form by 7:00 a.m. every morning.  Elementary students must have the health assessment form submitted by 7:30 a.m for the AM group. For the elementary PM group, please complete the form by 11:30 p.m.  When your child arrives at school, school personnel will check to make sure their form has been submitted. If your child does not have a submitted form, your child will not be allowed to enter the school building, and the school will reach out to parents/guardians to ask them to complete the form.

If you select “yes” for any of the questions in the form, please have your child stay home, report your child as absent to your school and call your medical provider. Students who stay home from school with COVID-19 symptoms will require a medical note to return to school.

If you have trouble with the web-based form, please print out a paper form, complete it manually, and send it with your child to school.
